Absence notification for Physics & Astronomy Students

Undergraduate students should use this form to inform the School of any absence from teaching sessions (lectures, seminars, lab classes, project meetings, exercise classes and so forth). All of the information that you provide through this form will be treated as confidential and will only be communicated to individuals within the School as detailed below.

Please note that funding authorities and prospective employers often require records of attendance from the School. In particular the School is legally obliged to pass attendance records for overseas students to the United Kingdom Border Agency (UKBA).

Please make sure to keep this notification on record. If the module organiser does not reply to your absence notification within five working days confirming receipt of your submission, please contact them personally.

Absence information

Please complete the reason for your absence in the below box. The text in this box will be sent to the Module Organiser, Deputy Module Organiser and, in some cases, the Deregistration Committee. If the Module Organiser or Deregistration Committee rejects your reason the absence will be marked as unauthorised, therefore you are asked to make your statement as clear as possible. If the reason for your absence relates to a personal or health matter that you would rather not detail on this form please make an appointment with your advisor (or Student Support Officer or other appropriate member of staff) to discuss the reason in person. If you do not disclose the reason in this box please note this below and list the name of the person with whom you will be discussing the absence.

For absences longer than five days, you must provide a medical certificate from an authorised medical professional. This medical certificate should be submitted to the Student Support Officer.
