Undergraduate students should use this form to inform the School of any absence from teaching sessions (lectures, seminars, lab classes, project meetings, exercise classes and so forth). All of the information that you provide through this form will be treated as confidential and will only be communicated to individuals within the School as detailed below.
Please note that funding authorities and prospective employers often require records of attendance from the School. In particular the School is legally obliged to pass attendance records for overseas students to the United Kingdom Border Agency (UKBA).
